  The only hub that we have that fits an Inner Bearing # LM67048 (1-1/4") and Outer Bearing # LM11949 (3/4") is the Trailer Hub Assembly # AH15450ECOMP. This is only available in a 4 on 5" lug bolt pattern. To get to the 5 on 4-1/2" wheel pattern you will need to source a wheel spacer that adapts from the 4 on 5" bolt pattern.

  I do have some options for you but we only offer two trailer hubs with these dimensions, the Dexter Trailer Idler Hub Assembly part # DX22XR which has a 5 on 4-1/2" bolt pattern and the Trailer Hub Assembly part # AH15450ECOMP which has a 4 on 5" bolt pattern.   I am afraid we do not carry any hubs that will fit your parameters. The .75" ID bearing is highly uncommon these days. We carry only one hub that uses a .75" Bearing with part # DX22XR.
