  Definitely looks like you have the Hayes 4 bolt style of 12 inch brake assemblies. So for the right hand side you'd want the part # 23-106-09 or for the left hand side part # 23-105-09.   If you have Dexter axles or any axle that uses the 5-bolt mounting flange, like # 4-44-1, then I would go with the tried and true Dexter assemblies, # 23-105 for the left side and # 23-106 for the right side.   Your dimensions are a little unique but I would assume you have an AL KO brand axle with a 6k axle and the correct assemblies would be the part # 23-105-09 and # 23-106-09. This is 12 inch in diameter assembly that can attach with a 4 or 5 lug bolt pattern.
