I walked out to our warehouse and pulled the Dexter 12 Inch Electric Brake Assembly # 23-105-09. It appears you do have a 12 inch brake assembly, and the measurements you provided match that of the Dexter Brake Assembly.

The # 23-105-09 is the left side brake assembly, for the right side of your trailer, you will need Dexter 12 Inch Electric Brake Assembly # 23-106-09.

This Dexter brake assembly will fit a 4-bolt brake mounting flange. Checkout the short video I produced for you showing the measurements of the Dexter 12 Inch Electric Brake Assembly # 23-105-09. Also linked is a video installation on replacing trailer drum brakes that may be of good reference.
