I checked with Reese and they have no hitch part 36069 showing on their website. I did find it elsewhere on the web but it is discontinued/out of stock. I checked with my contacts at Reese and was able to find out that this item fit various Chevy, Dodge and Ford vehicles from the late 70s through the late 90s. It does not fit any truck after model year 1999.
If you intend to re-install this hitch on a compatible vehicle please note you will need a new hardware kit # RHK. Hitch hardware cannot be re-used. On this item's page just enter the part number and we can supply a new hardware kit.
If you like I can still help you with confirmed trailer hitch fits for your 2004 Chevy Silverado 1500 4x4 that are currently available.
Using the drop-down menu tool on the linked page you can choose your 1500's bumper type to then see hitches that are compatible. For instance, if you have a 10-inch deep-drop bumper you can use Curt hitch # 13339 or for an aftermarket roll pan bumper hitch # 13932. Both of these are USA-made products.
