I reached out to Fastway and they stated that the hardware in this kit is not made in the USA. The origin was not specified, but they did say it comes from a country overseas.   Since the Replacement Bolt Kit # FA92-02-9600 has lock nuts with it you most certainly can use split ring washers with the flat washers. My contact at Fastway confirmed this. If you are having troubles with the conical washers coming loose you may want to ensure that you have the 3/4" shank bolts torqued to 250 ft-lbs as is specified on pages 20-21 of the installation instructions.

  According to Fastway, the 3/4 inch plated bolts they use are grade 5, but you'll want to use 3/4 inch conical toothed washers and lock nuts. The lock nuts can be found locally, but not the washers and Fastway won't honor the warranty if hardware other than what they recommend is used. If the guy buggered up the nut and bolt, I'd make him replace them using hardware from their # FA92-02-9600 hardware kit since he and not you damaged it.

Fastway doesn't offer them as an individual replacement part, but they are included with the # FA92-02-9600 replacement hardware/service kit. Without washers that get a good bite, you might lose the preload on the bolt, so you'll want to check the torque on them frequently until you can get them replaced.
