Is there a way to have a hitch and still foot operating the door hatch?

The liftgate sensors on your 2022 Toyota Sienna will be partially blocked by the Curt Trailer Hitch Receiver # C22UR but you will still be able to use them. It will take a little bit of practice to see where exactly the sweet spot is but you'll be able to move your foot around both sides of the receiver tube to find it.
