The only difference between the Dexter Electric Trailer Brake Assembly part # 23-181 for right hand or part # 23-180 for left hand and the Dexter Nev-R-Adjust Electric Trailer Brake Assembly part # 23-465 for right hand or part # 23-464 for left hand is the adjustment. The former are manual adjust brakes and the latter are self-adjust brakes. The manual adjust brakes require you to adjust them for even pad wear every so often whereas the self-adjust do this automatically. The adjustment procedure entails crawling on the ground under the trailer and using a screwdriver to spin the star wheel inside the brakes from an access hole in the rear. It's not hard to do, it just takes a bit of time and is something you'll need to remember to do every so often. The star wheel basically pushes the brakes out, closer to the drum for more contact as the pads wear down.

These brakes are both direct fits for Dexter, Lippert (LCI), and AL-KO 7,000 lb axles and measure 12" diameter.
