Since your 2012 Ford E-350 has a service body your best option for a hitch is with a weld-on unit like the Buyers Products Class 5 # 3371801050L. This particular unit includes the necessary mounting plates and has more than enough capacity for whatever the E-350 itself can handle.
Because of the modifications at the rear that a service body design requires, a standard custom (bolt-on) hitch won't be compatible.
Another good option, depending on how long the service body extends rearward, is the Buyers Products Class 4 Trailer Hitch # 3371801055. This unit features a longer 2" receiver to help with providing the needed clearance with the rear bumper on vehicles like yours.
