2009 Forest River 39FLFB Lodge w-super slide. Bad switch is 3 wire w- LCI on switch plate.

Thank you for the photo, that made finding your replacement much easier. You need the Lippert Components Hydraulic Interior Switch # LC120639, which as you can tell from the accompanying photo, is a direct replacement.

The Lippert Components Slide-Out Switch Assembly - White #LC117461 or black #LC117460 will work well for you. I've attached the wiring color coding in a photo to assist in your rewiring.
