I have the above vehicle with a multi tow adapter 7 blade / 4 prong hookup already installed. What parts do I need to hook up a controller?

You did not say if you already have a brake controller, so if you do not I recommend a brake controller like the P3, part # 90195. This is a proportional brake controller that applies the brakes on your trailer with the same intensity as the braking action of your 1997 Chevrolet pickup.
Since you already have a 7-way trailer connector on your truck you will just need to test the 7-way pins for function. Have someone sit in the driver seat and run through the functions while you check the pins on the connector with a circuit tester like part # 3808. Once you have identified all of the functions for the pins you will just need to complete the wiring for the brake controller as shown in the attached drawing.
As you can see in the drawing, circuit breakers are used for the power wire coming from the 7-way and also from the power wire coming form the brake controller. For the brake controller you can use a 30-amp breaker like part # 7012A, and if you need the 40-amp breaker you can use part # PK54540.
