This is a great question and one that we actually get quite often. Basically the Curt A20 5th Wheel Hitch # C16034 is the upgraded version of the Q20 # C16530-16017 and it's the one I specifically recommend.
The upgrades in question focus mostly on the head design. The A20 features an interlocking 2-jaw system as opposed to the standard 2-jaw system on the Q20. This interlocking mechanism allows the jaws to completely wrap around the king pin for a better connection and a smoother, quieter ride. If you clasp your hands together and interlock your fingers (as opposed to just connecting them at the fingertips) this gives you a pretty good idea as to the additional support and strength this design offers.
The Curt A20 also uses a fully articulating torsion head compared to the spherical axial bearing on the Q20. Both of these features help to create a really comfortable ride while towing, however the axial bearing on the Q20 makes it incompatible with pivoting pin boxes like the Lippert Turning Point, though not with the Sidewinder specifically. This just kind of limits your options if you ever wanted upgrade to this style of pin box or purchase a new trailer with one already installed at the factory.
Both options provide the same 20,000 pound towing weight capacity. In my opinion the Curt A20 is the clear winner between the two models. I have attached a short video review that you can check out as well.
