The replacement lift bracket that you will want to use with your Reese V5 Weight Distribution (WD) system is part # RP21501. If you need new chains you can use either part # 55630 or part # 3216. My contact at Reese confirmed these fits.

When choosing a WD system for your towing setup you will need to make sure that the Total Tongue Weight (TTW) of your setup falls in the middle of the Tongue Weight (TW) range of your WD system. You can find your TTW by adding the TW of your fully loaded and ready-to-go trailer along with the weight of any cargo behind the rear axle of your tow vehicle. Keep in mind that a properly loaded trailer will have a TW that is 10-15% the weight of the loaded trailer.

Since your current V5 WD system is rated for a max of 750 lbs and just the TW of your trailer is 1,200 lbs you will need to get a new WD system. The V5 that you have will not be effective on a heavier trailer.

I recommend going with the Reese Strait-Line WD System as it uses the chain hangers along with sway control cams which work to prevent sway instead of correct it after it has already started. If your loaded TW alone is 1,200 lbs then I recommend part # RP66075 which has a TW range of 1,000 lbs - 1,700 lbs. It doesn't come with a shank so you will need to use either # RP54976 or # RP54977. You will also need to use a Hitch Ball with a 1-1/4 inch shank. The Curt # C40085 is a great option because it can handle the higher weight ratings.

I have also linked some review videos and helpful articles that you can check out.
