The Curt # C15320 that you referenced should fit your 2006 GMC Savana with a Knapheide Body if your mounting holes have a 40 inch center. Although the outside measurement is relevant, the more important measurement is the mounting hole center to mounting hole center which is a standard 40 inches.
Providing you do have a 40 inch center, the Draw-Tite # 75189 will also fit.
Your step bumper is going to be the deciding factor when trying to pick out a hitch for your truck. If your step bumper is the standard factory bumper like the picture and you have a mounting hole measurement of 40 inches, then, either the Curt # C15320, Draw-Tite # 75189, or Reese # 45007 should fit.
You said that your frame rails were 41.75 inches to the outside - I pulled the Reese hitch and it has the same outside measurement that you are requesting - 41.75 inches and it has a 40 inch center. The Curt # C15320 and Draw-Tite # 75189 both have an outside measurement of 42-1/2 inches and center mounting holes of 40 inches.
In the picture, I have all three hitches installed on vans with factory step bumpers. If you happen to have a step bumper that is lower than the picture, the hitch will, more than likely, not fit on your vehicle.
With the variety of service vehicles that Knapheide assembles, I do not have a reference point as to which vehicle you have so I cannot verify your step bumper.
You will want to check out all 3 product pages because they have some differences, for example, the Curt and the Reese are Class V whereas the Draw-Tite is a Class III. With that simple difference it changes your tow ratings. Of course, you will have to check your owners manual to see what the tow rating is for your vehicle and choose from there. The other main difference is the Curt and the Draw-Tite have 2 inch receivers whereas the Reese is a 2-1/2 inch unit.
