The bolt pattern that you referenced doesn't match any of our current Fulton Jacks. I reached out the my contact at Cequent who makes the Fulton jacks and they said that they do not have a replacement for that bolt pattern. If you don't want to replace your weld-on bracket, I recommend the Fulton Bolt-Thru Swivel Marine Jack w/ Caster - Sidewind - 10" Lift - 1,200 lbs, # FXP10. This is a Bolt-Thru mount providing the minimum deflection and maximum support. It has the Z-Max 600 zinc finish that provides superior corrosion resistance.

In order to determine the torque rating for the lug nuts on your trailer you will need to know check out the chart I attached that shows all of the torque specs for all the trailer lugs available based on stud size, wheel material and wheel size.
