How do I mount the the HP99VR fender?

Single axle fender # HP99VR is a weld on application, so it is intended to be mounted by welding it directly to the side of the trailer using a wire feed or stick style weld.

I would recommend using a certified welder in your area.

I went out to the warehouse and measured the Mounting Brackets item #CE26061G-2 you referenced. The bolt hole spacing on the side with two holes is 3-1/2". The diameter of the bolt holes is 3/8". I have linked a diagram I made below.
