On each side of the hitch the rear most hole will use a square block spacer and a U- shaped spacer. The hole closest to the cab on each side will use a square block spacer on each side of the frame rail so one will be sandwiched between the hitch and the frame.   If you are considering a frame mounted hitch over your bumper ball mount/hitch I would recommend you go ahead and go the frame hitch route. This will give you quite a bit more towing capacity and the ability to use a different rise/drop ball mount so that you can tow your trailer as level as possible.
