No 5/8th bolts &nuts included
The product is as advertised. Solidly built and fits the frame well. However, there are a few issues I had with this order. First the item arrived unpackaged and is scratched and has deep scratches in areas. Second the receiver tube is not straight. It deformed slightly when it was welded apparently. The product is heavy duty and I think it will work ultimately but quality control in fabrication and shipping is below par on this order.

You are correct in that this hitch plate does not come with mounting hardware. To install the hitch plate it should be bolted with two five eighths Grade 8 bolts on each side and professionally welded all around the outside of each frame rail with a minimum weld size of five sixteenths.
