As far as hitches that are compatible with spare tires, all hitches we carry for your 2020 Acura MDX are only compatible with spare tires up to 18 inches. With that being said, I recommend the etrailer Trailer Hitch Receiver - Custom Fit # e98840. The great thing about the etrailer hitch (in addition to the economic price point) is the 12-inch clearance from the bottom of the hitch to the ground. This clearance will help avoid scraping or contact with any accessories you mount on the hitch. Also, the etrailer hitch installs only 3 inches from the face of the rear bumper to the center of the hitch's pinhole. With that measurement, you will be able to use a wider variety of accessories without worrying about bumper clearance issues.
