Does the axle flip void insurance coverage?

Trailers pretty often come from the factory with the springs over the axle as well as axles over the spring. Either variation is not inherently more dangerous than the other so having installed an over under kit like the part # K71-384-00 would not void insurance on a trailer.

In order to determine the torque rating for the lug nuts on your trailer you will need to know check out the chart I attached that shows all of the torque specs for all the trailer lugs available based on stud size, wheel material and wheel size.
