The most probable cause for not getting a display when you apply the truck brake, but getting a display when manually activate the override is a bad connection in the stop light circuit. The stop light circuit is the red wire coming off the brake controller that goes to the cold side of your brake pedal switch. It may still be working on the trailer in part because the inertia sensor may still be working in the brake controller.

This hitch may fit with your Tommy Lift, but you will need to compare the measurements shown in the diagram to your vehicle with the Timmy Lift to see if you will have enough clearance.

Although it isn't an exact match from the pictures you showed me, the Bargman LED Clearance/Side Marker Light - 59 Series # 47-59-002 is the closest we have and it does fit the same measurements. It's a 4 inches by 2 inches amber marker or clearance light with a white base. Also, the mounting holes are 3 inches apart, however on this light they are covered by the lens. In my experience a truck like your 2018 Isuzu NPR will either have a lift gate or a receiver. This is because the lift gate has to go up and down on the frame and is typically folded where the receiver will sit. Depending on how everything is mounted and the length of your chassis you may be able to still use a receiver like the Curt # C59CR with an Extension # C45048. All vehicles built after 1996 should have OBD II instead of the OBD ports, so your 2003 Isuzu N-Series will should almost certainly have an OBD II port.
