What size gooseneck ball will this unit fit? Are there any special requirements for the 5th wheel hitch in order to effectively connect on this adapter?

The Curt X-5 Gooseneck-to-5th-Wheel Rails Adapter Plate # C16210 that you referenced is designed to fit Curt Double-Lock flip and store hitches. C-60 and C-600 series hitches.
The gooseneck ball size does not matter, what matters is what hitch you have since this unit installs by removing the gooseneck ball and sliding into the hitch.
The only requirements for hitches that will fit this are that they are compatible with industry standard rails and have capacities less than 20k. Curt, Draw-Tite, Hidden Hitch, Reese, Valley and Husky hitches are all compatible.
