It sounds like you're looking for a hinge like the Ramp Door Hinge # PLR1902. This heavy-duty hinge replaces a damaged hinge on your trailer's ramp or access door. It has a steel construction and grease zerk fitting to easily grease it for easy operation. The bracket measures 4" long x 2-5/8" wide; x 1/4" thick and the pin is 8-3/16" long x 1" diameter. This has a raw finish and is made in the USA.

The entire hinge is 8-3/16 inches long by 1 inch in diameter. If you take a look at the photo I've edited for you, you can see all the dimensions of the Polar Steel Ramp Hinge, part #PLR1902.

The Ramp Door Hinge #PLR1902 does require welding of both portions. The tab gets welded as you stated to one side and the outside cylinders get welded to the opposite side. I've attached a couple pictures to assist.

The ramp hinges part #PLR1902 don't have a specific weight rating, but I can tell you that by the sheer size of these things and their steel constructions that three of them would be just fine for a 4,000 lb rated ramp.
