Does this item fit the 2020 Ram 3500 with the factory installed Gooseneck prep package. Curt 4 Offset Ball for Double-Lock Gooseneck Hitch Item # C602

I may have another option for you, but the Curt 4" Offset Ball for Double-Lock Gooseneck Hitch # C602 you mentioned will not work in the factory gooseneck in your 2020 Ram 3500. The factory gooseneck uses ball bearings to keep the ball in place, and due to that design there is nothing to keep the ball from rotating so an offset ball is not available.
The only option would be to install the B&W Extend-A-Goose Gooseneck Coupler - 2-5/16" Ball - 10" Offset - 24,000 lbs # BWTEXA4200 on your gooseneck trailer with will give you 10" additional clearance.
If you do not already have a gooseneck ball that is compatible with your Ram OEM gooseneck hitch I recommend the Curt Ball and Safety Chain Loops Kit for Ram Underbed Gooseneck Hitch - 30K # C60618.
