We have just what you're looking for. I recommend using the Curt # C11127 because of the heavier powder coating on the Curt hitches compared to others. This is a Class I hitch with a 1-1/4" receiver that is a confirmed fit on your 2012 Mazda 5.
For a wiring harness I recommend using the Tekonsha T-One Wiring Harness # 118520, which will tee into the plugs for your vehicle lights and provide you with a 4 way plug for trailer lighting. You will also need a ball mount similar to the Curt # C45147, which comes with a hitch ball. It's possible that you'll need a ball mount with a rise/drop, so I've also linked an article on how to measure appropriately for your trailer and determine which will fit best for your towing setup as well as all of our 1-1/4" ball mounts.
