K-Summit Lug Nut 21/22 mm Adapter Will Not Tighten Down

I have a 2014 Toyota Prius with 21mm lug nuts. I have tried all of the Thule adapter sizes, 17mm, 19mm, and 21/22mm, but not seem to fit properly. The 21/22mm adapter seems to slip off each time, even when fully tightened. Is there a 21mm only adapter available?

When we installed the K-summit chains here and we couldn't get the lug nut adapters like part # TH1500052581 to tighten down it usually meant that we needed to use the short 35 mm bolt or the longer spacer that came with the snow chain hardware. If you switch out one of those two or both it should allow you to tighten the adapter.
