The Curt Trailer Hitch, # C15320, is a Class V (class 5) hitch rated for 16,000 pounds gross trailer weight and 2,400 pounds tongue weight. It can also be used with weight distribution bringing the capacity totals up to 17,000/2,400 pounds.
It is designed to fit the following vehicles:
1996 - 2015 Chevrolet Express Van Full Size
1996 - 2015 GMC Savana Van Full Size
So, it is a fit for your 2004 Chevy Express Van. The recommended ball mount is # BWBMHD30010 and you can use pin and clip # PC3. For a ball, you can use # A-90 (2 inch diameter) or # 19286 (2-5/16 inch diameter). For trailer wiring you can use Tow Ready Custom Fit Vehicle Wiring, # 118392.
