For a 6-Pole Round connector, you would use adapter part # PK12717. Most of the manufacturers use 7-Pole connectors, not 6. There are 6 flat pins arranged in a circle with a large center post as the 7th contact. In this case, you would use part # PK12716.

I may have a solution for you, but it does not appear that there are any true air bag kits available for the 2009 Chevy Avalanche LTZ; from what I can find with some research it appears that most of these models have a shock and coil suspension and not a leaf spring suspension system.
