Dose this fan have the same mounting points as the factory fan shroud?

The Flex-A-Lite radiator fan part # FLX295 is the correct size and is a fit for your 2004 Toyota Sequoia but it is not an OEM replacement so it does not use the exact same mounting points of old fan. You will have to use the included mounting hardware to come up with best way to mount it. The brackets this comes with is the part # FLX29000.

Each of the fans of the Flex-a-lite Dual 13-1/2" Electric Radiator Fans w/ Shroud #FLX295 can flow up to 2,300 CFM. Also worth mentioning is that the likelihood of one of the fans failing is very low.
