Based on what you described it sounds like you are looking for the part # MA10-04301k-v16 which is a two switch setup attached to a circuit board for MaxxFan roof vents like what you have.

Yes the Replacement Wall Control Panel for MaxxAir MaxxFan Plus Roof Vent #MA59FR is correct for the MaxxAir fan 00-07500K fan. The fan comes with an RJ11 style of wire that you'd run to the wall switch #MA59FR.

You can use an RJ11 cable with the #MA59FR but some of the functions might not work properly. We've found that sometimes some of the functions will work backwards. What your best option would be is the communication cable part #MA69FR.
