I have a 28ft gooseneck with Dexter 10k axles. How can I tell which brake setup I have the 3 3/8 or 4 inch shoes?

To determine which brake assembly you need, measure the width of your brake linings, as shown in the photo I edited for you.
If your existing brake shoes measure 4 inches wide, the # 23-438 you mentioned would be an example of a good replacement for you if your existing brake assemblies use 7 mounting bolts (some use 4) and the diameter of the assembly is 12-1/4 inches in diameter when measured from the outside of one brake shoe directly across to the outside of the other shoe as shown in the photo. If this is indeed the correct replacement assembly for you, you would also need the right hand assembly, # 23-439. You will also need the mounting hardware which is not included with the assembly. You will need 7 each of the following for each brake assembly:
Mounting bolt, # 7-5
Split lock washer, # 38LW
Nut, # 38N
If your existing brake assembly uses the 7 bolt flange, and the brake shoes measure 3-3/8 inches wide, you would need # 23-450 for the right side and # 23-451 for the left. The same mounting hardware mentioned above would apply.
