I spoke with my contact at Reese regarding your used model 30004 fifth-wheel hitch that you recently purchased. He stated that this is an older model that used unique base rails specific to the hitch. Unfortunately, this particular unit is not compatible with industry-standard, above-bed base rails and the specific rails it needs are no longer available.
For a compatible hitch and rail kit for your 1995 GMC C/K Series truck, I recommend looking at the Reese 16K 5th Wheel Hitch # RP30047 along with the Universal Base Rails and Installation Kit # RP30035. This hitch features a 16,000 pound towing capacity and a 2-jaw locking system to provide excellent jaw-to-king-pin contact. The 4-Way pivoting head will ease hookup on unlevel ground and it also has an adjustable height from 14-1/2 to 18 inches.
I have attached a short video demonstration on this system that you can also take a look at.
