I checked with Kat's and the state that the correct plug to use on your 2004 Dodge Diesel Ram 2500 5.9 liter is the one in the RS position which is on the front corner of the engine block on the driver side.

On the label for your axle there will be the capacity listed. If you see just 12k or 12,000 lb axle then the #K2R1258L12 would work. If you see 12k HD or heavy duty then it would not.
