how often should one grease the Equalizer bolt?

It depends on how much use the trailer gets. If you do not use it very often then it is not working as much and the grease will not break down as quickly. So you will want to grease it at least once a year.
If you do use the trailer frequently you might want to check it a few times a year or when you notice the ride quality has changed because the suspension is not moving as freely as it used to.
