It sounds like you're looking for the Ventline Exterior Wall Vent which comes in Black # V2111-55 or White # V2111-13 to best match your application.
These are designed to fit a 10-1/2" wide x 3-3/4" tall rough opening and they come with the damper to cut off the opening from any small creatures. You can use the screws and sealant that comes with the Installation Kit # CAM25013 to complete the replacement.
