Trailer shows connected but at times, sh shows up. Could it be that the trailer and the vehicle are not grounded through the connection? Should it add a ground wire directly from the vehicle frame to the trailer frame?

The s.h error code you are getting on your Tekonsha Primus IQ # TK90160 means that it is detecting a short, and would not be caused by a bad ground. Since you mentioned this is happening when the trailer is connected I recommend starting with checking the vehicle and trailer connectors for any corrosion or debris that can cause a short.
If those check out you will want to check the brake output wire going from the trailer connector to each brake magnet on the trailer for any damage or sections where it could be getting pinched causing a short.
You will be want to be very through in checking the wires for damage, and replace any damaged sections with # 10-1-1 and heat shrink butt connectors # DW05745-5.
I included videos of the products mentioned for you to take a look at.
