With an order of a trailer access door do thumb locks have to be bought and installed separately or is it possible to swap them out for the order

If you're ordering an access door like the Ultra-Fab Universal Access Door for Trailers and RVs - 5" x 5" Opening - Chrome # UF48-979010 or the Ultra-Fab Universal Access Door for Trailers and RVs - 5" x 5" Opening - White # UF48-979009 then you'll have to order the Camco Cam Lock - Straight or Offset - Thumb Operated - 7/8" Thick
Item # CAM44323 separately to replace the keyed locks that come with the door.
