The 2-inch Curt Class III hitch # 13328 that is confirmed fit on your 2003 Acura MDX will be fully compatible with a full-size spare tire. I reviewed the installation instructions (which are linked for you) and also checked with my technical contact at Curt to confirm. On some models some trimming of the spare tire fascia will be required but the tire itself will not pose any fit concerns.

I have linked an installation video to show you the process (on a 2003 Honda) and you can refer to the linked instructions for a list of the tools required.

If you plan to tow you may need a wiring harness for trailer lights; if so you can use kit # 118336 which offers simple plug-and-play installation with no splicing of wires required. A video is provided for this item too. For a ball mount and 2-inch hitch ball you can use part # C45036 which includes the required pin and clip.
