The 40-45 psi preferred water pressure for the Fogatti RV Tankless Water Heater # LSB64FR has to do with the input pressure, not the output pressure. Installing a low flow shower head shouldn't be keeping the Fogatti from igniting since it would be the same as barely turning on a faucet. The trickle you get out of the head is in fact to avoid too much pressure to the hose and fittings going to the shower head from the main hot and cold knobs. When the water is turned off at the knobs the water will not trickle from the head and shouldn't unless one or both of the knobs is not completely closing off the water flow. Basically, a vacuum breaker has a piece of plastic that is held in place by water pressure. If the water pressure is to drop the piece will spring out of the way exposing a vent that will allow air to enter.

When the temperature of the water pipe in the water heater is lower than 43°F the controller on your # LSB64FR will display “Fd”. Your unit is going into frost protection mode to avoid getting damaged. It's very possible that the thermocouple could have gone bad, which is a fairly common cause for a water heater not to light. We do have a few replacement thermocouples available, such as the 18-inch # CAM09273, which are all universal-fit items.
I also recommend checking to make sure there are no loose connections anywhere and that everything is securely attached.

The Furrion RV Tankless Water Heater, # FR93FR uses a vortex system mixing the heated water with cold to eliminate fluctuations and reduce the risk of hot water scalding. It sounds like you may need to adjust the water control valve. I have linked the manufactures User Manual below which has a note listed to ensure your inlet water pressure is not greater than 65 psi.
