How to Transport 333-lb Wheelchair on 2016 Jeep Wrangler Unlimited with Rear Spare Tire

I need a wheelchair carrier for a 2016 Wrangler unlimited. The wheelchair is a Permobil C400. 36L X 26W. Its pretty heavy. But the Jeep has the spare wheel on it. I guess I need an extension??? With the extension, how is the tongue weight affected?
