That is an interesting problem you are experiencing. First thing you want to do is test the 4-way that is installed on the 2004 Xterra for voltage on the correct pins with a circuit tester like # 3808 like the video I attached.

I'm pretty sure there just isn't enough draw for the T-One # 118670 on your 2013 Nissan Pathfinder to recognize that it needs to send a signal. I called Tekonsha and they said they have had one other customer with this issue while using the T-One Vehicle Wiring Harness part # 118778 on a 2019 Honda Ridgeline. This sounds like a ground problem. What you can do is start by check the trailer side and vehicle side trailer connectors. You will want to look for any dirt or corrosion in or behind the trailer connectors. Corrosion can cause any number of lighting issues.

Like you said, for some reason some Nissan vehicles have those blue and black wires switched. You would have to swap their positions to keep the trailer brakes from being locked all the time.
Normally, the black wire is the 12 volt circuit and the blue wire is output to the trailer brakes. There should not be constant power out to the brakes or they will lock up.

So basically the brown of your trailer should run to the black of the light, the green of your trailer should run to the stop/turn signal, and the white should run to the white.
