If you are only getting 4 volts on the output side of wiring harness # 118496 then there are a couple of things to check. First, check that the ground is attached to a clean and corrosion free bare metal surface. No paint or undercoating either. Also, it works best if it is grounded by itself and not with other items.

If this does not help then you will want to test the converter. To do this use your meter to test the voltage going in the converter and then the voltage coming out. If you have the correct voltage (around 12 volts) going into the converter but only the 4 volts on the output side then the converter is bad and will need to be replaced.

But if the voltage is low on the input side as well then there is an issue with the vehicle wiring that will need to be fixed. I have included a couple of links to the right that should help you troubleshoot.
