DO YOU HAVE TO GROUND INPUT AND BOTH OUTPUT SIDES TO WORK?

The Hopkins Plug-In Simple Vehicle Wiring Harness item # HM43315 has one ground wire that needs to be attached to a clean source of metal. You will want to use the included self tapping screw and make sure you install it on a paint and rust free spot near your rear tail lights. It won't function without being grounded.

The visible metal portion of the 4-flat harness is the ground that your trailer connector will attach to. Your trailer lights will of course need to be grounded for your lighting system to function correctly.

I don't see what type of vehicle you have but the Hopkins Plug-In Simple Vehicle Wiring Harness item # HM43315 fits the following vehicles:

1989 - 1992 Toyota Pickup
1993 - 1995 Toyota Pickup excluding T100
1996 - 2004 Toyota Tacoma
1995 - 1995 Toyota Tacoma 1995.5 Mid-Year model

I have linked the installation instructions below.
