First you will need 2 of the Kodiak Disc Brake Kits # K2R1258L12 to switch from drum brakes to disc brakes on your 12,000lb Lippert general duty axle. If your Diamond C Trailer has the heavy duty axles please let me know, and if you are unsure send me a picture of the axle tags on each axle.
I recommend installing the Hydrastar Vented Marine Electric Over Hydraulic Actuator # HS481-8067. This is a plug and play system that will allow you to connect your current 7-way connection for a simple install. It also includes a CAM allowing you to use it with factory brake controllers.
The Hydraulic Brake Line Kit # HS496-252 contains the hoses for running to the brakes on a tandem axle trailer. The main line is 25' to go from the electric over hydraulic axles back to the front axle. If you have a longer trailer we do have a Union Fitting # BH-C-3FIF that will allow you to add an additional hole like the 6' Hose # bh-3mfs-6
