Based on the information you have provided I recommended the Fastway e2 Weight Distribution w/ 2-Point Sway Control # FA92-00-0800. This system has sway control built into the hitch and uses trunnion bars to not reduce your ground clearance as much as round bar systems. One measurement that you didn't provide is the tongue weight. This weight distribution is rated for 400-800lbs of tongue weight and given your 5,000lb loaded rating with 10-15% tongue weight that puts you from 500-750lbs of tongue weight. You can always use a Trailer Tongue Weight Scale # e99044 to verify your tongue weight.

You will need to select either a 2-5/16" Hitch Ball # EQ91-00-6100 to match your trailer coupler. A 1-7/8" 12pt Thin Walled Socket # ALL643216 will be needed to install the hitch ball.
