Based on the bearing numbers you provided, the Dexter Hub and Drum Assembly, # 8-201-5UC3, is most likely the correct hub for your spindles. This hub comes with inner bearing # 25580 and outer bearing # LM67048. It was designed for 5,200 lb axles and has a 6 on 5-1/2 inch bolt pattern.
To be absolutely certain that this hub will work for you, I recommend checking the grease seals on your hubs as well. This hub comes with a # 10-1 grease seal. If possible, check your old grease seals for their part number. It should be stamped into the rubber of the seal. If you do not see a part number, you can measure the diameter of your spindle where the grease seal sits (see photo).
This hub accepts 12 inch by 2 inch brake assemblies like the Dexter 12 Inch Brake Assembly - Left Hand, # 23-105, and Right Hand, # 23-106. To determine if these brake assemblies will fit on your axle, you will want to measure the bolt pattern of your brake mounting flange. These brakes require a 5 bolt brake mounting flange. I have attached a dimensions photo showing the bolt hole spacing that your brake mounting flange should have to use these brakes.
If your brake mounting flange has a different bolt pattern, let me know and we can make a more specific recommendation for you.
