You can most definitely use the Thule Helium # TH44VR with a front-mounted trailer hitch receiver on your 2018 Toyota 4Runner. There are just a few things you will want to keep in mind when mounting this bike rack on the front end of your vehicle. It could possibly limit headlight visibility along with the possibility of restricting airflow to the radiator; this likely will not be a big issue but is something to think about.

If you do not have a trailer hitch receiver yet then I recommend going with the CURT Front Mount Trailer Hitch Receiver - Custom Fit - 2" # C31054 if you have the Limited model, or go with Curt Front Mount Trailer Hitch Receiver - Custom Fit - 2" # C31076 if you have the SR5 or TRD model. Both have the same 350lb tongue weight and allow fitment of any 2" accessory such as the Thule Helium bike rack.
