Do we combine the weght and say carrying capacity of 12,000 lbs?

The hub you mentioned, # 42655UC1 is compatible with a 6,000 lb capacity axle. Each hub is rated at 3,000 lb max, or 6,000 lb per axle. For a tandem axle set-up, the hubs would provide a 12,000 lb capacity.
Assuming that your existing hub uses a # 25580 inner bearing, and a # 15123 outer bearing, this would be the proper replacement. Four hubs would equal a 12,000 lb capacity for a tandem axle trailer.
These hubs are compatible with grease seals that use either a 2.25 (#RG06-070) or a 2.125 inside diameter (# RG06-090), although the # 42385 is included with the hub. If you need the # RG06-090 seal, it would need to be ordered separately.
If you cannot read the reference number on your existing seal, you can use a digital caliper on the surface of the spindle that the seal rides on to determine which diameter is needed.
