The best way to add clearance when towing with a truck that has it's tailgate down is to go with a longer ball mount like the # D-26 that you referenced which is overall 12-5/8 inches. You can use hitch extenders but they end up cutting the capacity of the hitch they are installed by 50 percent so they aren't my favorite solution to recommend.

The longest fixed ball mount we carry is the Hidden Hitch # 80232. It's 15-3/4 inches from the hitch pin hole to the ball hole. It provides 3/4 inches of rise or 2 inches of drop.

The Brophy # BH20 is a bit longer at 13-1/4" from hole to hole and we also have the Hidden Hitch # 80232 that takes it up a notch, measuring 15-3/4" long. This represents the longest ball mount we have available.
