The difference between the two brake assemblies comes down to one being self adjusting and one that is manually adjusted. I recommend the self adjusting because you will never have to crawl under the trailer to adjust the brakes. This can be a headache and we tend to not do it as often as we should which leads to poor performing brakes. With the self adjusting it automatically tightens as the shoes wear down keeping your trailer's braking performance optimized and working as safely as possible.

Part # 23-181 is the Dexter Electric Trailer Brake Assembly - 12" - Right Hand - 7,000 lbs this assembly requires periodic adjustments be made to the brakes.

Part # 23-464 is the Dexter Nev-R-Adjust Electric Trailer Brake Assembly - 12" - Left Hand - 7,000 lbs this assembly doers not require you to adjust them.

The left side for the non adjusting is Part # 23-180
The right side for the self adjuster is Part # 23-465

It is recommended that you replace these in pairs so you have equal performance from one side to the other so you have no issues with the trailer darting on you towards the stronger brake's side.

For this you can use Part # 23-464-465 which is the self adjusting left and right side assemblies or Part # 23-108-181 for the non adjusting left and right assemblies.

I've included videos on each of them to help explain the difference and an installation video Ryan did to show that process. Happy Hauling!
