In order to adjust the hitch head height of the B&W Companion OEM 5th Wheel Trailer Hitch part # BWRVK3670 you will need to remove the pivot arms and then reinstalled them using either the top, middle, or bottom set of bolt holes. I have attached a portion of the install instructions that go over this process for you.

If the provided hitch head height adjustments do not allow you to tow your trailer level then you may need to raise the trailer. For this, we have the Tandem Axle Lift Kit part # RR23QX or the Dexter Trailer Springs Over-Under Conversion Kit part # K71-385-00 (leaf spring axles only).

With the B&W hitch on a Ram 3500 with a 6.4 bed, pulling a 35' Keystone Montana. Will I need to remove the tailgate when hauling or install a V-Tailgate? Thanks

You don't necessarily need to remove or swap out your tailgate - it all depends on the profile of your 5th wheel and the location of the pin box. There isn't a set of rules or anything to go by when it comes to this, it's literally going to be you see how everything interacts with each other. Typically the only time people will remove or swap out the tailgate is when they don't want to have to mess with it when hooking up and unhooking from the trailer.
